Contains patient's name, number of pregnancies, date admitted, diagnosis, age, description of condition, treatment and subsequent report, date of death, sometime includes name of examining doctor or surgeon. Also contains watercolour illustration by JPSW of case of 'Epithelioma Labia minora' Feb 1920. Indexed by surname of patient. (1 volume)
