Blank samples of CAPS form, current and lifetime diagnosis version, used in 1994-1996 follow-up study of survivors of the 1988 sinking of the cruise ship JUPITER, comprising questions on the frequency and intensity of: unwanted memories of the traumatic event; distress caused by events that symbolised or resembled the trauma; vivid hallucinations or flashbacks; distressing dreams of the event; deliberate avoidance of thoughts or feelings concerning the trauma; avoidance of activities and situations that were reminders of the trauma; psychogenic amnesia; reduced interest in activities; feelings of detachment; numbing of emotions; conviction of impending death; difficulty falling or staying asleep; anger; difficulty concentrating; hypervigilance; exaggerated startle response; physical reactions to recollection of trauma; lasting sense of guilt; murderous feelings; disillusionment with authority; timing and duration of worsening symptoms; impact of symptoms on social and professional life.
